Report a Service Incident to Information Technology Services (ITS)

On this page the ITS Service Management and Operations team provides the campus technical community the ability to report a potential service outage or degradation, to begin the Incident Management process, before work is started to find a solution.

Any incident that has a noticeable service impact of a customer group requires an incident notification. Here are our definitions:

  • Incident — Any unplanned event interrupting a normal IT service operation, ranging from degradation of service to a complete outage.
  • Degradation — A decline in normal IT service levels, which does not entirely interrupt the IT service.
  • Outage — Any period of complete disruption or interruption of an IT service.

All fields are required, unless otherwise noted.
